Matériel et méthode : Les coeurs de dix sujets ont été prélevés à la recherche de cellules myocardiques dans les
veines pulmonaires. Des coupes histologiques ont été réalisées sur 39 VP.
Résultats : Des cellules myocardiques ont été mises en évidence dans les VP de 9 à 38mm de distance de la
jonction veino-atriale. Ces cellules se répartissaient en une couche circulaire externe et une couche longitudinale
interne. En périphérie, l’organisation de ces cellules était plus irrégulière. C’est dans les VP supérieures que l’on
retrouvait des cardiomyocytes le plus à distance de la jonction veino-atriale. A cette jonction, la couche
circulaire n’était pas toujours circonférencielle.
Conclusion : La structure des parois des veines pulmonaires renforce l’hypothèse de l’initiation des FA à ce niveau.
Cette idée prend toute son importance pour le traitement des FA par radiofréquence.

Objective: Atrial Fibrillation (AF) is the most common abnormal heart rhythm and contributes to the cardiac
morbidity and mortality. Electrophysiology studies with pacing showed that AF was initiated by ectopic beats
localized in pulmonary veins (PV) walls. The aim of this work is to look for some anatomic or histological
particularities to explain these ectopic beats.
Methods: Ten autopsied hearts were examined (6 males and 4 females). The myocardium was studied from the left
atrium to the PV. Histological sections were made fron thirty nine PV.
Results: Muscular cells localized into PV between 9 to 38mm from PV-atrial junction. The sleeve was
composed of circularly and longitudinally oriented bundles of myocytes. The peripheral end of the myocardial
sleeve was irregular. The longest myocardial sleeves were found in the superior veins and they were
longitudinally oriented. At the PV-atrial junction, the circularly bundles were not often circomfered ;
Conclusion: The myocardial architecture in PV sugested the possibility to initiate the AF. This fact is important for
radiofrequency ablation.
