- i think the key phrase here is not "primary stroke" but "primary prevention", i.e. "primary stroke prevention" = "primary prevention of stroke". my hunch is based on your context above and internet searches on stroke prevention yielding primary and secondary preventions instead of 'primary stroke' per se.
- karyanet-perubatan gives "primary prevention" as "pencegahan primer". [pls see url link below and also note that karyanet-perubatan also has an entry on "secondary prevention" as "pencegahan sekunder".]
- of the two choices you provided, "pencegahan utama strok" may be a viable alternative. but i'm in favour of going with karyanet-perubatan.
