This is a report on an elderly patient who died.
I want to know how you would translate 'insufficience', here, given that it can (I believe) be translated either as 'insufficiency' or 'failure'. What is the difference? Does 'cardiac failure' always mean that the patient dies? Here, he is known to have ..., so I have translated 'insufficience' as 'insufficiency'. Does this sound OK, or would it be better to use, for example, 'weakness', or something else - or 'failure'?

Oh, I see now - for some reason (it's late), I saw the instance with "mitral" without seeing the first instance with cardiac.

I agree with Drmanu - "heart failure" for the first instance, and "insufficiency" - or one of the other options you mentioned - for the second instance with "mitral."

Note that heart failure can be chronic, i.e. non-fatal (while still a very serious condition). See, for example, "Living well with chronic heart failure" at (PDF):

Asker: There are two uses of 'insufficience' in this sentence: with 'cardique' and with 'mitrale'. I found many more references to 'regurgitation', as far as mitral is concerned, but I'm not sure if that means the same as 'insufficiency' or not, or if both are commonly used in English. There is also the term 'mitral incompetence', which appears to mean the same thing (but might not). I think, probably, any of these three terms would do, with mitral (although possibly not). However, with 'cardiac', I was not sure if it would be better to use 'failure', or if that meant that the person died, This is further complicated by the fact that this patient did die, but, previously to that, he was known to have 'insufficience cardiaque'.
