Preface
This book stems from the authors' involvement in a three year project, researching autism and Asperger syndrome in Lancashire. During that time, the authors met over 100 children with Asperger syndrome, in a variety of settings. The content of the book draws on this breadth of experience.
The anecdotes in the book are all based on real children, but names have been changed for reasons of confidentiality.
The authors met many parents of children with Asperger syndrome, but one deserves a special mention: Lynda Bannister, mother of John, who writes so eloquently and positively of her son's strengths and difficulties. We are pleased to have the opportunity to quote directly from her writings. Lynda stands as a representative of all the parents and children from whom we have learnt so much.

Asperger Syndrome: An Introduction
Who was Asperger?
Hans Asperger (1906 1980) lived and worked in Vienna. He qualified as a doctor and specialised in paediatrics. His work brought him into contact with a number of boys who found it difficult to 'fit in' socially. In addition to their poor social interaction skills, the boys had difficulties with the social use of language, together with a limited ability to use and understand gesture and facial expression. Also evident were repetitive, stereotypical behaviours, often with 'abnormal fixations' on certain objects.
Having noted the similarities in the behaviour of a number of these boys, Asperger (1944) wrote and presented his paper 'Autistic psychopathies in childhood'. He recognised how severely the boys' difficulties affected their everyday lives, commenting, 'they made their parents' lives miserable and drove their teachers to despair'. He was also aware of the boys' many positive features they often had a high level of independent thinking, together with a capacity for special achievements but he didn't underestimate the impact of their individuality on others with whom they came into contact, and he noted their vulnerability to teasing and bullying.
Asperger's paper was written in German towards the end of World War II and for this reason reached only a limited readership. It only became widely accessible in the early 1980s when it was first translated into English and referred to by Lorna Wing in her own research into autism and related conditions. It was felt that the term 'Autistic psychopathy' sounded too negative, and 'Asperger syndrome' was suggested as a more acceptable alternative.
Autism and Asperger syndrome
At the same time as Asperger was doing his research in Vienna, the child psychiatrist Leo Kanner was working in Boston, USA. He saw a similar cluster of behaviours in a number of children whom he went on to describe as 'autistic' using the same descriptor which Asperger had used for his research group. Both Kanner and Asperger had referred to the work of Bleuler (1911) when choosing the word 'autism'. However, Bleuler had used the term to describe children who had withdrawn from participation in the social world. Kanner stressed that the children he was describing had never been participants in that social world, whilst Asperger felt that the coining of the word 'autism' was 'one of the great linguistic and conceptual creations in medical nomenclature'.
For Kanner, 'early childhood autism', on which he wrote his (1943) paper 'Autistic disturbance of affective contact', had a number of defining features, including:
• a profound autistic withdrawal;
• an obsessive desire for the preservation of sameness;
• a good rote memory;
• an intelligent and pensive expression;
• mutism, or language without real communicative intent;
• over sensitivity to stimuli;
• a skilful relationship to objects.
Later researchers, particularly Lorna Wing (1981b and 1991), compared Asperger's writings to Kanner's early papers and noted significant similarities between the children being described. The key difference was that the children described by Asperger had developed grammatical speech in infancy although the speech they had was not used for the purpose of interpersonal communication.
The core difficulties in autism and Asperger syndrome are shared. Asperger syndrome involves a more subtle presentation of difficulties. This is not to say that it is a mild form of autism as one parent said, 'My child has mild nothing.' Asperger syndrome affects every aspect of a child's life and can cause great upset for the family.
A commonly held view is that Asperger syndrome should be regarded as a sub category of autism part of the wider spectrum, but with sufficient distinct features to warrant a separate label. This view is useful for educational purposes as it is generally accepted that intervention and treatment approaches for children anywhere within the autism spectrum will share the same foundation.
The term Asperger syndrome is useful in explaining to parents and teachers the root of the many problems they encounter with a child who is intellectually able, yet experiences significant social difficulties.
The triad of impairments in autism
While Asperger's paper lay undiscovered, Kanner's observations on the nature of autism were the subject of much discussion, debate and further research. Lorna Wing and Judith Gould (1979) carried out an extensive epidemiological study in the London borough of Camberwell. They concluded that the difficulties characteristic of autism could be described as a 'Triad of Impairments'.
They emphasised the fundamentally social nature of the three linked areas of difficulty:
• impairment of social interaction;
• impairment of social communication; e impairment of social imagination, flexible thinking and imaginative play.
Wing and Gould noted that there were many children who did not exactly fit Kanner's description of 'early childhood autism', but who, nevertheless, had significant difficulties within the areas of the triad. This led Wing (1981a) to use the term 'Autistic continuum' and later (Wing 1996) 'the Autistic spectrum', allowing for a broader definition of autism based on the triad.
Diagnostic criteria
There is acceptance that autism is characterised by the co occurrence of impairments in social interaction, social communication and social imagination, and diagnostic criteria are agreed on the basis of the triad.
In 1981, as a result of examining Asperger's original paper, Lorna Wing (1981a) outlined the following criteria for Asperger syndrome:
• impairment of two way social interaction and general social
ineptitude;
• speech which is odd and pedantic, stereotyped in content, but which is
not delayed; e limited non verbal communication skills little facial expression or gesture;
• resistance to change and enjoyment of repetitive activities; e circumscribed special interests and good rote memory;
• poor motor coordination, with odd gait and posture and some motor stereotypies.
Whereas Asperger maintained that speech was acquired at the normal age, Wing disagreed. From her own experience, she found that half the population she would describe as having Asperger syndrome had not developed language at the normal age. Criteria developed by Christopher Gillberg (1989) were broadly similar.
Two major diagnostic instruments are currently in use by clinicians the Diagnostic and Statistical Manual, 4th edition (DSM IV, American Psychiatric Association 1994) and the International Classification of Diseases, 10th edition (ICD10, World Health Organisation 1992) see the appendix. Both systems base their diagnostic criteria for Asperger syndrome on the three fundamental impairments outlined within the triad. Following Asperger, they rule out early language delay, and neither includes motor coordination difficulties as a diagnostic feature.
Numbers of children with Asperger syndrome
In 1993, Stefan Ehiers and Christopher Gillberg published the results of research which attempted to establish the prevalence of Asperger syndrome. It had been carried out in Gothenburg and involved studying children in mainstream schools. From the numbers they identified as having Asperger syndrome, they calculated a prevalence rate of 36 per 10,000, having used criteria which allowed for the presence of some early language delay.
All prevalence studies have indicated that boys are far more likely to be affected than girls. Asperger himself had felt that it could be an exclusively male difficulty. Gillberg (1991) suggests that the ratio of boys to girls is in the region of 10:1.
Causes
As yet, the cause of Asperger syndrome is unknown. It is unlikely that there is a single cause rather a set of triggers, any one of which, occurring at a certain time within a chain of circumstances, can cause Asperger syndrome.

Asperger thought that the condition was probably transmitted genetically, describing it as an 'inherited personality disorder'. Although current thinking is that Asperger syndrome is not directly inherited, continuing research looks at the possibility of some genetic basis.
Today, Asperger syndrome is described as a brain dysfunction, and researchers are trying to pinpoint the area or areas of the brain in which the dysfunction occurs. As technology improves, it may be possible to indicate this more precisely.

Prefácio
Este livro tem como objectivo, em primeiro lugar, dar a conhecer as experiências do autor, recolhidas ao longo de três anos, durante os quais foi realizado um projecto em Lancashire, visando a pesquisa sobre a síndrome de Asperger. Durante este período, o autor entrevistou mais de cem crianças portadoras desta sindrome, em circunstâncias variadas. Os relatos são verídicos e baseiam-se em crianças reais, no entanto, os nomes foram modificados por questões de confidencialidade.
De entre os pais e mães entrevistados pelo autor, uma merece menção especial, Lynda Bannister, mãe de John, que escreve de um modo muito eloquente e positivo sobre as dificuldades e fraquezas do filho. Sentimo-nos gratos por poder tirar proveito do que ela escreveu. Linda tornou-se assim uma porta-voz de todos os pais de crianças que sofrem desta sindrome, através das quais tanto aprendemos.

A Sindrome de Asperger: uma introdução
Quem Foi Asperger?
Hans Asperger (1906 1980), viveu e trabalhou em Viena. Tirou o curso de medicina, vindo posteriormente a especializar-se em pediatria. O seu trabalho fez com que viesse a contactar mais de perto com rapazes que pareciam ter dificuldades em se enquadrar socialmente. Para além da inadaptabilidade social, estes pareciam ter ainda dificuldades no que dizia respeito à linguagem verbal, bem assim como limitações na compreensão de gestos e expressões faciais. Asperger pode também verificar um padrão de comportamentos repetitivos e estereotipados, muitas vezes com uma fixação anormal num único objecto.
Não podendo deixar de notar a similaridade no comportamento de muitos destes rapazes, Hans Asperger escreveu e publicou uma dissertação em 1944, intitulada: “Psicopatia Autista na Infância”, afirmando então que esta doença parecia afectar os rapazes em todas as etapas da sua vida diária, “tornando a vida dos pais num verdadeiro inferno e levando os professores ao desespero”. No entanto, estava também consciente das suas características positivas, a sua capacidade de pensar independentemente, bem assim como atingir metas específicas, não subestimando no entanto o impacto do seu individualismo sobre aqueles com quem conviviam e a sua vulnerabilidade a “bullying” e troça por parte dos colegas.
A dissertação de Asperger foi publicada em alemão, nos finais da Segunda Guerra Mundial, contando apenas com um número reduzido de leitores. Viria a ter um maior impacto nos anos oitenta aquando da sua tradução para inglês, sendo então referida no trabalho de pesquisa da Drª Lorna Wing, sobre autismo e problemas subjacentes. Acreditava-se, na época, que o termo “psicopatia autistica” tinha uma conotação demasiado negativa, sugerindo-se então a expressão “Síndrome de Asperger” por parecer mais adequada.
Autismo e a síndrome de Asperger
Paralelamente ao trabalho da pesquisa de Asperger, o psicólogo Leo Kanner desenvolvia em Boston, nos Estados Unidos, um trabalho de investigação sobre a síndrome de Asperger. Também ele encontrou um leque de comportamentos similares em crianças que ele viria a descrever como “autistas”, usando os mesmos padrões descritivos que Asperger havia utilizado na sua dissertação. Tanto Asperger como Canner, usaram o termo “autista” nas suas descrições, que fora usado por Heller, no seu trabalho publicado em 1911. No entanto este último havia-o utilizado para descrever crianças que se tinham desenquadrado do mundo social. Canner fez questão de salientar que essas mesmas crianças nunca participaram nesse mesmo mundo, enquanto que Asperger considerou que o termo “autismo” era uma das grandes criações linguísticas e conceptuais na nomenclatura médica.
Para Canner, o autismo infantil, sobre o qual escreveu um artigo em 1943, “Distúrbios Autistas no Contacto Afectivo”, tinha na sua opinião um número de características definidas, entre as quais se incluíam:
• Um afastamento profundo do convívio social;
• Um desejo obsessivo de preservação de comportamentos repetitivos;
• Uma boa memória intelectual
• Uma expressão pensativa e inteligente;
• Mutismo ou linguagem sem uma verdadeira intenção comunicativa;
• Hiper-sensibilidade a estímulos
• Um relacionamento muito hábil com objectos.
Os investigadores das gerações seguintes, particularmente Lorna Wing, (1981b and 1991), compararam o que Canner havia escrito com os artigos de Asperger e notaram semelhanças muito significativas entre as crianças que ambos descreviam. A diferença principal entre ambos era o facto de as crianças descritas por Asperger terem desenvolvido uma linguagem discursiva gramatical na infância, embora não a usassem com o propósito de comunicar inter-pessoalmente.
No entanto, as dificuldades quer do autismo quer da síndrome de Asperger são semelhantes, apresentando porém a síndrome de Asperger um leque de dificuldades mais subtis, o que não quer dizer que sejam menos complexas que as verificadas no autismo propriamente dito. Como um familiar afirmou: “O meu filho não tem quase nada”. A síndrome de Asperger pode afectar todos os aspectos da vida de uma criança e causa um grande desgosto à família.
A opinião geral é que a síndrome de Asperger deverá ser vista como uma sub-categoria do autismo, parte de um contexto mais alargado, tendo no entanto, características individuais definidas, o que lhe confere portanto uma designação diferente do autismo. Esta opinião é útil para fins educativos, já que se considera que se devem aplicar os mesmos métodos tanto diagnósticos como pedagógicos a crianças autistas e com a síndrome de Asperger. O termo síndrome de Asperger é útil como modo de melhor poder explicar aos pais e professores a causa profunda do problema que irão encontrar numa criança que, é perfeitamente capaz intelectualmente, mas que sente grandes dificuldades em se enquadrar socialmente.
As deficiências do autismo
Enquanto o artigo de Asperger estava ainda por descobrir, as pesquisas de Canner sobre o autismo deram lugar a debates acalorados e a um estudo epidemiológico no distrito londrino de Camberwell, desenvolvido, posteriormente, pelas investigadoras Lorna Wing e Judith Gould (1979), Concluíram assim que as dificuldades do autismo poderiam ser descritas como “uma tríade de deficiências”. Também elas fizeram questão de enfatizar a natureza fundamentalmente social das três áreas afins em que as crianças mais dificuldades sentiam:
• Interacção social;
• Comunicação social, ;
raciocínio social, fluidez de pensamentos e capacidade de imaginação
Wing e Gould verificaram, no entanto, que em várias crianças não se podiam aplicar os padrões descritivos usados por Canner para designar o “autismo na infância”; contudo estas apresentavam muitas das deficiências descritas por elas. Isto fez com que Wing em 1981 passasse a utilizar o termo continuo??? e mais tarde a mesma Wing, em 1996, viesse a utilizar o termo “espectro autista” como um conceito mais alargado, baseando-se no entanto nas teorias descritas anteriormente.
Critérios De Diagnóstico
Crê-se que no autismo há uma co-ocurrência de dificuldades a nível social, comunicativo, assim como imaginativo e social, e há um consenso quanto aos critérios de diagnóstico, baseados na “tríade” descrita por Wing e Gould, em 1981,
Depois de um estudo dos artigos originais publicados por Asperger, Lorna Wing designou os seguintes critérios para a síndrome de Asperger:
• Dificuldades tanto de inter-acção social como sociais de modo geral
• Inadaptabilidade
• Discurso pedante e estereotipado não sendo, no entanto, lento
• Capacidade linguística verbal limitada, bem assim como poucas expressões faciais e gestos
• Resistência à mudança e satisfação na prática de actividades repetitivas ; interesses circunscritos e uma boa memória
• Coordenação motora insatisfatória, com postura estranha e estereótipos motores.
Enquanto Asperger considerava que a fala era adquirida na idade normal, Wing discordava. No seu trabalho de campo, pode verificar que metade da população, que poderia descrever como sofrendo da síndrome de Asperger, não tinha desenvolvido capacidades linguísticas na idade normal. Os critérios desenvolvidos por Christopher Gillberg em 1989, eram de modo geral semelhantes.
Dois métodos de diagnose estão actualmente a ser utilizados pelos médicos: o “Manual de Estatística e Diagnose”, 4ºa edição, publicado pela Associação Psiquiátrica Americana em 1994, e a “Classificação Internacional de Doenças”, 10ºa edição, publicada pela Organização Mundial de Saúde, em 1992, (ver o apêndice). Os dois manuais baseiam os seus critérios de diagnóstico, quanto à síndrome de Asperger, nos descritos na “tríade de dificuldades”, anteriormente mencionada. Tal como Asperger, ambos descuram tanto as dificuldades de linguagem tardias, como dificuldades motoras, como critérios de diagnóstico.
Número de Crianças com a síndrome de Asperger
Em 1993, Stefan Ehiers e Christopher Gillberg realizaram e publicaram uma pesquisa, cujos resultados pretendiam estabelecer a prevalência da síndrome de Asperger. Fora realizada em Gotenburgo e teve como objectivo o estudo de crianças integradas em escolas normais. Do número de crianças identificadas como sofrendo da síndrome de Asperger, calcularam uma taxa de trinta e seis em cada dez mil, usando, no entanto, critérios que incluíam a linguagem verbal tardia.
Todos os estudos com relevância concluíram que os rapazes têm uma maior probabilidade de sofrer desta síndrome, do que as raparigas. O próprio Asperger considerou que esta era uma deficiência exclusivamente masculina. Gillberg, 1991, sugere que a relação rapazes/ raparigas é de dez para um.
Causas
Tanto quanto nos é dado saber até ao momento, a causa da síndrome de Asperger é ainda desconhecida. É provável que não haja uma causa única, mas antes um conjunto de factores que podem ser desencadeados a certa altura, provocados por uma cadeia de circunstâncias ocasionando, assim, a síndrome de Asperger.

Asperger pensou que esta doença era transmitida geneticamente, descrevendo-a como “uma desordem de personalidade hereditária”, embora se acredite actualmente que a síndrome de Asperger não seja de ordem hereditária, continuando no entanto a desenvolver-se pesquisas, no sentido de detectar a possibilidade de alguns factores genéticos poderem ter certa influência.
