The dough time is considered to be the length of time following the start of mixing for the mixture to achieve a dough-like mass that does not stick or adhere to the walls of a polypropylene mixing beaker and can be removed in one piece using a spatula. The set time is determined by forming the dough into a sausage shape and occasionally tapping it against a hard surface. The set time is considered to be the time from the start of mixing to the point at which the mixture is transformed into a hard mass that does not deform and gives a noticeable change in the sound produced when tapped against a hard surface. The expected median OS of the control arm of xxxxx, aaaaa and placebo from the initiation of blinded therapy is 10 months. A 3.5 month improvement of OS for the treatment arm of xxxxx, aaaaa and zzzzz (median OS: 13.5 months) translates to a true hazard ratio (HR) [aaaaa, xxxxx and zzzzz regimen vs aaaaa, xxxxx and placebo regimen] of 0.74. Assuming an exponential distribution, approximately 640 subjects will be randomized in a 1:1 ratio and will be followed until 465 deaths are observed to ensure 90% power with a Type I error rate of 5% based on a 2-sided log-rank test. It is expected that the accrual will take approximately 17 months and the study will take 35 months to complete. This design includes one interim OS analysis after 395 deaths have been reported for superiority only and the final analysis after 465 deaths. Superiority will be declared at the interim or at the final analysis if the nominal p-value reported by the stratified log-rank test is less than 0.01 for the interim analysis, and 0.0494 for the final analysis. A group sequential testing approach with Haybittle-Peto alpha spending function is used.
A significant percentage of long term survivors have been reported in clinical studies that evaluated immunotherapy. As a consequence, the survival curves in the study may not follow an exponential decay and there may be a flattening of the death rate curve at the time close to the end of this study. Should such a phenomenon occur, it would result in prolonged study duration. In order to mitigate this potential phenomenon, the following stopping rule for the final analysis of OS will apply:
In the situation that the expected number of deaths has not reached the 465 target but 35 months have elapsed since the first subject was randomized, the final analysis will occur according to the following rules:
• If the target number of 395 deaths for the planned interim analysis of OS has not been reached by 35 months from the time the first subject was randomized, the final analysis will be performed after 395 deaths are reported with a full alpha spending of 0.05. In this situation, the interim analysis of OS will be cancelled.
• If the target number of 395 deaths for the interim analysis has been reached prior to 35 months post randomization of the first subject, the final analysis of OS will be performed at the earlier of the occurrence of 465 deaths or 41 (35 6) months from the time the first subject is randomized.
