A large proportion of right-hemisphere stroke patients exhibit unilateral neglect, a neurological condition responsible for many debilitating effects on everyday life, poor functional recovery, and decreased ability to benefit from treatment (Held et al., 1975, Denes et al., 1982, Fullerton et al., 1986, Katz et al., 1999, Jehkonen et al., 2000 and Jehkonen et al., 2006). Two main theoretical trends may be distinguished in the rehabilitation of neglect: a “top-down” and a “bottom-up” approach (see Fig. 1, for reviews see Rode et al., 2003, Luauté et al., 2006a and Luauté et al., 2006b). More recently, new systemic transversal approaches have also been developed, targeted at non-spatially lateralised deficits frequently associated with neglect. In the framework of the present review, we will focus on one of the most promising “bottom-up” strategies, i.e., prism adaptation (PA).

PA to a lateral displacement of the visual field is known to directionally bias sensory-motor correspondences. One major question about the effects of sensory-motor plasticity in general and of PA in particular is about their specificity. While zero or poor transfer to unexposed effectors or movement speed have been described in healthy subjects (e.g. Redding and Wallace, 1988 and Kitazawa et al., 1997), therapeutic results obtained in neglect patients suggest that visuo-manual adaptation can surprisingly produce effects at numerous levels of this condition. Crucially, effects of PA have thus been described for non-motor and non-visual tasks, suggesting that the effects of PA extend to unexposed sensory, motor and cognitive systems.
We will introduce our review by briefly summarizing the classical data and issues about visuo-motor adaptation to prisms in healthy participants. Then we will focus on the available clinical data substantiating the fascinating expansion of prism adaptation effects on neglect manifestations (this term will be defined below). Then we will consider behavioural and neural arguments allowing us explaining the expansion of prism adaptation to cognitive variables. This section will also further outline the pathophysiological model proposed by Pisella et al. (2006).The following section will provide an inventory of the crucial technical and clinical parameters to consider for designing optimal prism adaptation protocols. Finally we will provide practical recommendations for the routinely use of prism adaptation in rehabilitation and for further investigations. The original goal of the present review will be to focus on the outstanding generalization and expansion of adaptation to the cognitive parameters of unilateral neglect, as understanding this peculiar property of prism adaptation constitutes the major scientific challenge in the field.
2. Sensori-motor adaptation and spatial cognition
2.1. A behavioural bias
In healthy subjects, a behavioural bias can be experimentally induced by a simple PA procedure (e.g. Welch, 1974, Rossetti et al., 1993 and Redding et al., 2005). It should be emphasized here that it has long been accepted that both the demonstration and the quantification of prism adaptation are obtained via the measure of after-effects—i.e. by the comparison of visuo-motor pre- and post-tests. The reduction of errors that can be observed during prism exposure does not in itself provide evidence for adaptation (e.g. Weiner et al., 1983). It is therefore unfortunate that recent studies do not respect this terminology (e.g. Fernandez-Ruiz et al., 2003). Following exposure to a rightward optical deviation of the visual field, subjects show a systematic leftward deviation of visuo-motor and proprioceptive responses with the adapted limb (see Fig. 2). The crucial observation is that these sensory and motor biases are opposite to those described in UN patients without adaptation (Jeannerod and Rossetti, 1993). Our original proposal (Rossetti et al., 1998) was to use these PA-induced sensorimotor alterations as a method to re-orient neglect patients behaviour towards the neglected side and tentatively amend their deficits. Another interesting feature of the sensori-motor re-organization induced by PA is that it can be produced without requiring patients’ voluntary attention to their left-sided deficit, i.e., according to a bottom-up process that bypasses their damaged awareness and intentional control (see Rode et al., 2003). As reviewed below, important and reliable therapeutical effects of right prism adaptation have been obtained in left neglect. The directional specificity in neglect patients has been demonstrated: none of the tested neglect patients showed a consistent effect of left-deviating prisms, neither in terms of sensorimotor adaptation nor in terms of visuo-spatial effects. In contrast to normal subjects, patients with left neglect adapted only to right-deviating prism and not to left-deviating prism.

1The strategic shift in motor responses occurs immediately, producing a rapid reduction in pointing errors made when the prisms are worn. This component relies mainly on the parietal cortex.
2The adaptive spatial alignment is developed after a more prolonged prism exposure, and is revealed when the prisms are subsequently removed. This negative after-effect is held to be characteristic of longer-term plastic changes in visuo-motor mapping. This component relies on the cerebellum.
Interestingly, experiments in healthy subjects suggested that the cognitive effects of prism adaptation depend also on the direction of the optical shift, with a pattern opposite to the one obtained in neglect patients (Colent et al., 2000, Berberovic and Mattingley, 2003, Michel et al., 2003, Michel et al., 2008 and Striemer et al., 2006): rightward deviating prisms generate only sensori-motor adaptation, while leftward deviating prisms may induce both sensori-motor and cognitive after-effects.
2.2. Measurement of PA
The basic PA procedure involves 3 steps: pre-exposure baseline measurement of pointing performance; active exposure to prismatic displacement to produce adaptation; and post-exposure after-effect measurement of adaptation persistence (Redding et al., 2005 and Pisella et al., 2006) (Fig. 2). Assessment of pointing performance before prismatic exposure provides an account of baseline outcomes. It allows quantifying prism adaptation after-effects.
Measure of midsagittal judgment based on different sensory afferences is frequently used: visual straight ahead (subject is asked to verbally indicate when a visual target shifting laterally is in a straight ahead position); subjective straight ahead pointing without vision (including motor and proprioceptive components and inappropriately depicted as “proprioceptive” midsagittal judgment); proprioceptive straight ahead (arm passive proprioceptive midsagittal judgment; Hatada et al., 2006); global measure of visual and proprioceptive realignment (pointing movement towards visual target without any vision of arm movement [open-loop pointing movement]).During the early phase of the exposure, error reduction alone (mainly accounted for by the strategic component) does not imply that prism adaptation is already effective. The pre and post-tests, optimally performed with non-exposed targets (Redding et al., 2005), are used to compute the amount of after-effect, i.e. proper adaptation. Adaptation is thus confirmed by a shift of open-loop pointing movements in the direction opposite to the optical shift (after-effects = post-test − pre-test). This total after-effect magnitude usually corresponds to the addition of visual and proprioceptive after-effects (Redding and Wallace, 1993). However the overall realignment amplitude is generally lower than visual deviation's one (40% in Redding and Wallace's study, 1993 [Redding and Wallace, 1993]; and see for example Rossetti et al., 1998, Rode et al., 2001, Pisella et al., 2002 and Maravita et al., 2003).
Other parameters have also been used (see Serino et al., 2006 and Serino et al., 2007): index of error reduction (comparison of pointing performance during pre-exposure and exposure condition) and index of after-effect (comparison of open loop pointing performance between post- and pre-exposure conditions), with no correlation between these two visuo-motor effects after PA.
2.3. Factors influencing prism adaptation
In healthy subjects, several parameters can modulate adaptation (see review in Redding et al., 2005): movement speed, presence or absence of visual feed-back (Freedman, 1968), realistic characteristic of the conflict (Norris et al., 2001), simultaneity between movement execution and visual reafference (Hay and Goldsmith, 1973 and Kitazawa et al., 1995). In neglect patients, systematic investigations of exposure conditions have not been yet conducted, and it remains uncertain that these different parameters might play a key-role in patients. One recent study (Làdavas et al., 2011) crucially compared the effects of two PA treatments, one based on terminal and the other on concurrent exposure procedure. The results suggest a better neglect improvement for patients treated with terminal exposure, correlated to a stronger error reduction effect. However, it has been also shown that PA therapeutical effect can be achieved using ecological activities (games) instead of laboratory pointing tasks (Shiraishi et al., 2008 and Fortis et al., 2010), which suggests that the potential contribution of terminal exposure is marginal.
Cognitive parameters may affect PA in contrasting ways. First, after-effects may be reduced by increased cognitive load (i.e., mental arithmetic or mental imagery, Redding and Wallace, 1985). Second, awareness of the visual disturbance before the adaptative procedure (Jakobson and Goodale, 1989) can also affect after-effects magnitude. Michel et al. (2007) specifically investigated the contribution of awareness for the optical shift to the development of prism adaptation. The use of a gradual exposure prevented subjects from becoming aware of the optical deviation and led to greater after-effects and robustness than in the classical single-step exposure. In the same way, when strategic corrections are reduced (e.g., physiological ageing, Fernández-Ruiz et al., 2000; pathological ageing, Weiner et al., 1983), proper adaptation has been found reinforced (predominance of adaptative component produces stronger after-affects). Interestingly, this crucial point has implications for neglect patients, who strongly depart from healthy subjects for their awareness of the optical shift. These patients are (pathologically) not aware of the optical deviation and exhibit larger and longer-lasting after-effects as compared to healthy controls. Taken altogether, cognitive parameters probably critically interfere with adaptation, with an apparent contradiction between the negative role of attentional load and the positive role of unawareness. The patients’ reduced awareness for the optical shift parameter may explain some of the discrepancy between patients and healthy individuals in terms of the sensitive parameters of exposure.
The relationship between different components of PA, the precise exposure conditions and the characteristics of different consecutive measures together highlight the complexity of contributing processes. According to the current body of data, what is true for healthy subjects seems to also apply for patients.

La négligence spatiale unilatérale est un trouble neurologique que beaucoup de patients développent à la suite d'un accident vasculaire cérébral de l'hémisphère droit. Elle est responsable de différents effets invalidants sur la vie quotidienne des patients, dont la récupération fonctionnelle est limitée et la prise en charge thérapeutique réduite (Held et al., 1975; Denes et al., 1982; Fullerton et al., 1986; Katz et al., 1999; Jehkonen et al., 2000, 2006). On peut distinguer deux tendances théoriques majeures dans la rééducation de la négligence : l'approche « top-down » et l'approche « bottom-up » (voir Fig. 1, voir pour revue Rode et al., 2003; Luaute et al., 2006a,b Plus récemment, de nouvelles approches systémiques transversales sont apparues, ciblant des déficits non spatialement latéralisés, mais souvent associés à la négligence. Nous nous concentrerons ici sur l’adaptation prismatique (AP), considérée comme étant l'une des stratégies « bottom-up » les plus prometteuses.

Il est établi que l’AP à un décalage latéral du champ visuel induit un biais directionnel au sein des correspondances sensori-motrices. Une des questions majeures concernant les effets de la plasticité sensori-motrice, de manière générale mais plus particulièrement avec l'AP, porte sur leur spécificité. Alors qu'aucun transfert ou presque n'a été observé chez les sujets sains vers un effecteur non exposé ou vers des mouvements de vitesses différentes (ex : Redding et Wallace, 1988; Kitazawa et al., 1997), les résultats thérapeutiques obtenus chez les patients négligents suggèrent qu’une adaptation visuo-manuelle peut étonnamment produire des effets à des niveaux très différents de ce trouble. Il est important de noter que les effets de l'AP ont ainsi été observés pour des tâches non motrices et non visuelles, ce qui signifie que les effets d’une telle procédure s'étendent à des systèmes sensoriel, moteur et cognitif non exposés.
Nous introduirons cet article par un bref résumé des données et des questions classiques concernant l'adaptation visuo-motrice aux prismes chez les participants sains. Ensuite, nous nous concentrerons sur les données cliniques disponibles afin d'étayer l'impressionnante expansion des effets de l'adaptation prismatique sur les manifestations de la négligence (le terme expansion sera défini plus loin). Nous examinerons par ailleurs les arguments neuraux et comportementaux qui nous ont permis d'expliquer l'expansion de l'adaptation prismatique vers des variables cognitives. Dans cette partie, nous approfondirons également le modèle physiopathologique proposé par Pisella et al. (2006). Nous poursuivrons avec un inventaire des paramètres techniques et cliniques essentiels à prendre en compte pour concevoir des protocoles d’adaptation prismatique optimaux. Pour conclure, nous fournirons des recommandations pratiques pour une utilisation courante de l’adaptation prismatique, aussi bien dans le cadre de la rééducation que pour mener des enquêtes plus poussées. L'objectif fondamental de cette revue sera de réfléchir aux exceptionnels phénomènes de généralisation et d'expansion de l’adaptation aux paramètres cognitifs de la négligence, puisque comprendre ces propriétés spécifiques à l’adaptation prismatique constitue l’enjeu scientifique majeur de ce domaine.
2. Adaptation sensori-motrice et cognition spatiale
2.1. Un biais comportemental
Chez les sujets sains, une simple procédure d'AP peut expérimentalement susciter un biais comportemental (par ex, Welch, 1974; Rossetti et al., 1993; Redding et al., 2005). Soulignons le fait qu’on a longtemps accepté l’idée selon laquelle on peut témoigner de la manifestation et de la quantification de l'adaptation prismatique en mesurant ses effets consécutifs, c'est-à-dire en comparant les pré-tests et les post-tests visuo-moteurs. Certaines études récentes considèrent à tort que la réduction d’erreur est synonyme d’adaptation et utilisent ainsi ces deux termes de la même façon (ex : Fernandez-Ruiz et al., 2003). Or, la réduction des erreurs observable au cours de l’exposition prismatique ne représente pas en soi une preuve d’adaptation (ex : Weiner et al., 1983). Après avoir été exposés à une déviation optique du champ visuel vers la droite, les patients présentent une déviation systématique vers la gauche des réponses visuo-motrices et proprioceptives du membre concerné (voir Fig. 2). L'observation essentielle est que ces biais sensoriel et moteur sont opposés aux biais constatés chez les patients négligents sans l'adaptation (Jeannerod and Rossetti, 1993). A l'origine, nous proposions (Rossetti et al., 1998) d’utiliser ces altérations sensori-motrices induites par l’AP comme une méthode pour réorienter le comportement des patients négligents vers le côté négligé, afin de compenser provisoirement leurs déficits. La réorganisation sensori-motrice engendrée par l'AP comporte une autre caractéristique intéressante : en effet, du fait de l’utilisation d’un procédé bottom-up, c’est-à-dire contournant le contrôle conscient et intentionnel lésé des patients, on peut obtenir cette réorientation sans engager leur attention volontaire vis-à-vis du côté gauche négligé (voir Rode et al., 2003). Comme nous le décrirons plus loin, l’adaptation prismatique droite a eu des effets thérapeutiques conséquents et fiables sur la négligence gauche. Par ailleurs, la spécificité directionnelle a été prouvée chez le patient négligent : quand les prismes dévient le champ visuel vers la gauche, ils ne produisent d’effet cohérent sur aucun des patients négligents testés, que ce soit en termes d’adaptation sensori-motrice ou d’effets visuo-spatiaux. Ainsi, à l’inverse des sujets sains, les patients négligents gauches ont montré une adaptation uniquement aux prismes déviant le champ visuel vers la droite, et non aux prismes déviant le champ visuel vers la gauche.

1La déviation stratégique des réponses motrices survient immédiatement, produisant une réduction rapide des erreurs de pointage faites durant le port des prismes. Cette composante est essentiellement reliée au cortex pariétal.
2Le réalignement spatial adaptatif est développé après une exposition plus prolongée aux prismes, et est révélé lorsque les prismes sont par la suite enlevés. Cet effet consécutif négatif est considéré comme une caractéristique des modifications à plus long terme des correspondances visuo-motrices. Cette composante est reliée au cervelet.
Curieusement, les expériences menées sur les sujets sains ont suggéré que les effets cognitifs de l’adaptation prismatique dépendaient également du sens de la déviation optique, mais selon un fonctionnement opposé à celui observé chez les patients négligents (Colent et al., 2000; Berberovic and Mattingley, 2003; Michel et al., 2003, 2008; Striemer et al., 2006). Les prismes déviant le champ visuel vers la droite n’entrainent donc qu’une adaptation sensori-motrice, alors que les prismes déviant le champ visuel vers la gauche peuvent produire aussi bien des effets consécutifs sensori-moteurs que cognitifs.
2.2. Mesure de l’AP
La procédure classique d’AP implique 3 étapes :
- la mesure pré-exposition des performances de pointage de référence ;
- l’exposition active à la déviation prismatique entrainant l’adaptation ;
- et la mesure post-exposition des effets consécutifs dus à la persistance de l’adaptation
(Redding et al., 2005; Pisella et al., 2006) (Fig. 2).
L’évaluation des performances de pointage avant l’exposition prismatique fournit un compte-rendu des résultats de référence, ce qui permet de quantifier les effets consécutifs de l’adaptation prismatique. On utilise souvent la mesure du jugement de l’axe sagittal, fondée sur différentes afférences sensorielles :
- le droit devant visuel (une cible visuelle se déplace de gauche à droite, et le patient doit indiquer verbalement quand la cible se situe droit devant lui) ;
- le pointage droit devant subjectif à l’aveugle (considéré à tort comme un jugement « proprioceptif » de l’axe sagittal, il implique des composantes motrice et proprioceptive) ;
- le droit devant proprioceptif (jugement proprioceptif de l’axe sagittal par un mouvement passif du bras (Hatada et al., 2006)) ;
- et la mesure globale du réalignement visuel et proprioceptif (mouvement de pointage vers la cible visuelle, sans aucune vision du bras [mouvement de pointage en boucle ouverte]).
Lors de la phase initiale d’exposition, une réduction d’erreur seule, principalement due à la composante stratégique, ne signifie pas que l’adaptation prismatique est déjà efficace. On utilise les pré et post-tests, effectués dans des conditions optimales sur des cibles hors exposition (Redding et al., 2005), pour calculer l’ampleur des effets consécutifs, c’est-à-dire l’adaptation à proprement parler. L’adaptation est ainsi confirmée par une déviation des mouvements de pointage en boucle ouverte vers la direction opposée à la déviation optique (effets consécutifs = post-test – pré-test). Habituellement, l’ampleur totale des effets consécutifs correspond à l’addition des effets visuels et des effets proprioceptifs (Redding and Wallace, 1993). Cependant, l’amplitude globale du réalignement est généralement plus faible que celle de la déviation (40 % selon l’étude de Redding et Wallace, 1993 [Redding et Wallace, 1993]; voir par exemple Rossetti et al., 1998; Rode et al., 2001; Pisella et al., 2002; Maravita et al., 2003).
D’autres paramètres ont également été utilisés (voir Serino et al., 2006, 2007), comme l’indice de réduction d’erreur (comparaison entre les performances de pointage avant et pendant l’exposition), et l’indice d’effet consécutif (comparaison entre les performances de pointage en boucle ouverte avant et pendant l’exposition), sans corrélation entre ces deux effets visuo-moteurs après l’AP.
2.3. Facteurs influençant l’adaptation prismatique
Chez les sujets sains, certains paramètres peuvent moduler l’adaptation (pour revue, Redding et al., 2005 : la rapidité du mouvement, la présence ou non de feedback visuel, les caractéristiques réelles du conflit (Norris et al., 2001), la simultanéité entre l’exécution du mouvement et la réafférence visuelle (Hay and Goldsmith, 1973; Kitazawa et al., 1995). Chez les sujets négligents, l’importance du rôle que peuvent jouer ces différents paramètres reste cependant incertaine puisqu’à l’heure actuelle, personne n’a encore mené d’enquêtes systématiques quant aux conditions d’exposition des patients. Récemment, une étude (Ladavas et al., 2011) a comparé de façon déterminante les effets de deux traitements par AP, l’un étant basé sur une procédure d’exposition terminale et l’autre sur une procédure d’exposition avec vision libre. Chez les patients traités par exposition terminale, les résultats démontrent une plus grande amélioration de la négligence, corrélée à un effet de réduction d’erreur plus important. Cependant, les résultats démontrent également que l’on peut obtenir l’effet thérapeutique de l’AP via des activités écologiques (jeux), plutôt que des tâches de pointage en laboratoire (Shiraishi et al., 2008; Fortis et al., 2010), ce qui signifie que l’éventuelle contribution de l’exposition terminale est minime.
Les paramètres cognitifs peuvent influer sur l’AP de différentes manières. Tout d’abord, les effets consécutifs peuvent se voir réduits par une augmentation de la charge cognitive (calcul mental ou imagerie mentale, Redding and Wallace, 1985). Par ailleurs, la conscience du trouble visuel avant l’adaptation (Jakobson and Goodale, 1989) peut également affecter l’ampleur des effets consécutifs. Michel et al. (2007) ont spécifiquement analysé l’impact de la conscience vis-à-vis de la déviation optique sur le développement de l’adaptation prismatique. Le recours à une exposition progressive empêchait ainsi les sujets de prendre conscience de la déviation optique, ce qui conduisait à des effets consécutifs plus importants et à plus de robustesse qu’après l’exposition brutale classique. De la même manière, quand les corrections stratégiques étaient réduites (par exemple, vieillissement physiologique, Fernandez-Ruiz et al., 2000 ; vieillissement pathologique Weiner et al., 1983), l’adaptation à proprement parler s’est trouvée renforcée (la prédominance de la composante adaptative produit de plus forts effets consécutifs). Curieusement, ce point décisif présente des implications chez les patients négligents, qui se distinguent fortement des sujets sains en ce qui concerne la conscience de la déviation optique. Ces patients ne sont (pathologiquement) pas conscients de la déviation optique et présentent donc de plus grands et de plus longs effets consécutifs comparé aux sujets sains. Quand ils sont réunis, ces paramètres agissent de façon probablement cruciale sur l’adaptation, avec une contradiction manifeste entre le rôle négatif de la charge attentionnelle et le rôle positif de la non conscience. Ce déficit de conscience vis-à-vis du paramètre de déviation optique chez les patients négligents peut expliquer certaines contradictions entre les sujets négligents et les sujets sains au niveau de la sensibilité des paramètres d’exposition.
La relation entre les différentes composantes de l’AP, les conditions d’exposition bien précises et les caractéristiques des différentes mesures consécutives viennent mettre en lumière la complexité des processus contributifs. Si l’on en croit le corpus de données disponibles aujourd’hui, ce qui est vrai chez les sujets sains semble aussi s’appliquer chez les sujets négligents.
