At ---- City nurseries, we request 670 yen each month as a school lunch fee (to cover rice and bread) for all class members 3 years and older. Discounts are available for those with allergies etc. Please see the head of the nursery.
The fee can be debited directly from your bank account.
How to apply for direct debit of the fee:
1. Select either the green form for the Japan Post Bank automatic payment service, or the blue form for transferring money from a financial institution other than the Japan Post Bank, and carry out the appropriate procedure at your post office or financial institution. For details, see the attached information on how to fill out the form and the sample.
2. After completing the procedure at your financial institution or post office, bring the authorized form to the nursery on the day of the health check.
3. The school lunch fee can be debited monthly, quarterly or in a yearly lump sum.
If you are debiting from a financial institution other than the Japan Post Bank:
After completing step 1 above, your financial institution will return the form for the payer and the form for ---- City to you. On the payer form, about half-way down the page, you will find payment method options. Please circle one and submit this to the nursery.
(The first page of the form for funds transfer is for the financial institution. Note that this form does not include the option to select payment method.)

Halfway down the form for the payer there is an option to select the payment method. Please circle one.
