Source text - English Medical Report Patient: XX Date of birth 01.01.1980 Hospital Treatment: 04 to 06 March 2005 Diagnosis: Atrial fibrillation with Tachyarrhythmia absoluta, now sinus rhythm Struma cervicalis nodosa Hypercholsterinemia History: The patient noticed a sudden pain in his shoulder during washing his feet. The doctor found a fast heart rate and a low blood pressure, so that the patient was referred to hospital. Clinical findings: Heart rate initially 170/min, irregular. No heart murmurs. Lungs normal. Blood pressure in admission 160/100 mmHg. Further examinations: ECG: Tachyarrhythmia absoluta, heart rate initially 170/min. No indication for an acute ischemia. Laboratory: slightly elevated liver enzymes. No elevated heart enzymes. CRP normal. Echocardiographia: slight left ventricular hypertrophy. Sonogram of thyroid: one node left lobe (6 x 7 mm), one node right lobe (6 x 8 mm). Stress-ECG: no sings of a stress-dependent ischemia. The thyroid hormones are being examined. The results are expected in about 2 weeks. Therapy and Development: After treatment with digoxine, beta blockers and anticoagulation spontaneous conversion into sinus rhythm. Follow up examinations of ECG and laboratory were normal. The patient felt well without any further symptoms. Recommendations: Medication: Beloc Zok (metoprolol) 1-0-0 Simvastatin as before Control of cholesterol and blood pressure. Szintigram of thyroid and follow up-ECGs in Italy. The patient should be released from duty for the next two weeks. he should be referred back to Italy by MEDEVAC and is able to fly without any special monitoring equipment. Terapia ed Evoluzione: Dopo trattamento con digossina, beta bloccanti e anticoagulanti passaggio al ritmo sinusale. Successivi esami ECG e di laboratorio, normali.
