Antihistamines are mainly used to treat seasonal allergic rhinitis (hay fever), urticaria (hives), pruritus (itching) and insect bites and stings. They may also be used to help reduce nausea and vomiting, and in the emergency treatment of anaphylaxis - a severe allergic reaction. The side- effects most commonly experienced are minor.
What are antihistamines?
Antihistamines are a class (group) of drugs used in the treatment of allergic disorders and some other conditions. They include: acrivastine, cetirizine, desloratadine, fexofenadine, levocetirizine, loratadine, mizolastine, alimemazine, chlorphenamine, clemastine, cyproheptadine, hydroxyzine, ketotifen and promethazine. These also come in various different brand names. They can be taken as tablets, syrups or nasal sprays, and some come in droplet form for use in the eyes.
What conditions are antihistamines used to treat?
Antihistamines are commonly used:
To relieve the symptoms associated with hay fever. These can include rhinitis and conjunctivitis (inflammation of the nose and eyes), sneezing, itching of the eyes, nose and throat, and rhinorrhoea (runny nose).
To reduce the severity of the rash and itching associated with nettle-type rashes such as urticaria and generalised pruritus (itch).
To prevent motion sickness and other causes of nausea (feeling sick).
In the care of the terminally ill, for their sedating and antisickness effects. In the emergency treatment of severe allergic reactions.
How do antihistamines work?
Histamine is a chemical naturally produced by various cells in your body. It has a variety of different functions. Large amounts of histamine are made in cells called mast cells in places where the body comes into contact with the outside environment. For example, in the nose, throat, lungs and skin. Here, mast cells and histamine form part of your immune defence system (whereas, in the stomach, histamine made by cells that line the stomach helps to produce acid for food digestion).
The cells of the immune system constantly monitor your blood and mucosae (membranes lining body cavities such as your mouth, nose and digestive tract) for anything that is not made by your own body. For example, germs such as bacteria or viruses. If your skin is damaged or your immune system detects a foreign substance, histamine is released from mast cells. The histamine binds to special receptors (sites) on other cells called H1 receptors. This sets off a chain reaction which causes blood vessels in the area to become slightly leaky.
Specialised cells and chemicals, which defend your body, can now get access to the area. While this is a helpful response, it also causes redness, swelling and itching.
Allergic reactions such as hay fever are caused by an oversensitivity or over-reaction of the immune system to a particular allergen. An allergen is a substance that is foreign to the body and which can cause an allergic reaction in certain people. For example, pollen, dander, mold, some germs. In most people, the immune reaction to these foreign substances is normal and appropriate. But in allergic people, it is excessive. For example, in people with hay fever, contact with pollen in the nose, throat and eyes triggers the mast cells there to release much more histamine than normal. This excessive release of histamine produces the associated symptoms of itching, swelling, runny eyes, etc.
Antihistamines work by physically blocking the H1 receptors, stopping histamine from reaching its target. This decreases your body's reaction to foreign substances (allergens) and therefore helps to reduce the troublesome symptoms associated with allergy.
Antihistamines are also used in the treatment of nausea and vomiting. However, the exact way that they ease these symptoms is not fully understood. The brain has several key areas which control vomiting. It is thought that antihistamines block H1 receptors in the area of the brain which creates nausea in response to chemicals in the body.
Some antihistamines may also have what is known as an antimuscarinic effect. This means that the drug can also block another type of receptor found on the surface of certain cells. If these receptors are affected, you may experience some of the side-effects associated with antihistamines, such as dry mouth, blurred vision and retention of urine. These effects are mainly caused by the older first-generation antihistamines which are described below.
Note: antihistamines should not be confused with H2 blockers which reduce the production of stomach acid. While both types of drugs block the actions of histamine, they work on different receptors in different systems of the body.
Are there different types of antihistamines?
Generally, antihistamines have been classified into two groups:
First-generation or sedating antihistamines can cause significant drowsiness and are generally more associated with the antimuscarinic side-effects mentioned above. These include: alimemazine, chlorphenamine, clemastine, cyproheptadine, hydroxyzine, ketotifen and promethazine. These drugs may be used for their sedative effects should your sleep be disturbed by itching.
Non-sedating or second-generation antihistamines are newer drugs which generally cause less
drowsiness. However, anyone taking these drugs while performing skilled tasks, for example driving, should be aware that a sedative effect may still occur and, in particular, in combination with alcohol. Second-generation antihistamines include: acrivastine, cetirizine, desloratadine, fexofenadine, levocetirizine, loratadine, mizolastine and rupatadine.
Can I buy antihistamines?
Some of these drugs are available to buy over the counter from your pharmacist. Others are only available on prescription.
Which is the best one?
All antihistamines work pretty well to reduce symptoms of allergy. Your doctor or pharmacist may advise or prescribe a particular antihistamine depending on the exact cause of your allergy and on whether you require a sedating or non-sedating drug. For example:
In general, antihistamines are probably roughly equally effective in reducing the symptoms of hay fever and urticaria. However, non-sedating antihistamines tend to be used more commonly as they cause less drowsiness.
Second-generation antihistamines are generally advised for most allergic situations as they cause less drowsiness.
Cetirizine, fexofenadine, or loratadine are often recommended for urticaria.
Once-daily preparations desloratadine and fexofenadine may be more beneficial in reducing the symptom of blocked nose - a symptom not classically controlled by the use of antihistamines. A sedating antihistamine may be particularly helpful for children at bedtime who have allergic symptoms.
How do I take antihistamines?
These drugs come in a variety of forms, as mentioned above. Your doctor or pharmacist will advise you on how to take your medication, including how often. Read the leaflet that comes with your particular brand for further information.
How quickly do antihistamines work?
An antihistamine tablet typically starts to work within 30 minutes after being taken. The peak of effectiveness is typically within 1-2 hours after being taken.
Antihistamines are generally more effective when taken constantly rather than intermittently. This is particularly applicable for people with hay fever. In the summer months the pollen count is generally higher and you may be in contact with the allergen on a regular basis. Taking the medication regularly will help keep your symptoms under control. Their effectiveness will also depend on the dose you take and what form the drug is given in.
How long is treatment needed?
This can vary depending on the reason for treating you. If you suffer from hay fever you may take the drug throughout the pollen season.
Who cannot take antihistamines?
Most people can take antihistamines safely. Antihistamines should not be used by people with acute porphyria (a rare metabolic disorder). In addition, they may not be suitable for people with liver or kidney problems or for pregnant or breast-feeding mums. First-generation antihistamines may not be suitable for men with benign prostatic hyperplasia (enlargement of the prostate) or people with acute glaucoma.
A full list of people who should not take antihistamines is included with the information leaflet that comes in the drug packet. If you are prescribed or buy an antihistamine, read this to be sure you are safe to take it.
What about side-effects?
Most people who take antihistamines do not have any serious side-effects. If side-effects do occur, they are usually minor. The most common are:
Drowsiness. Headache. Dizziness. Agitation.
Dry mouth. Blurred vision.
Urinary retention (difficulty passing urine). Gastrointestinal discomfort (stomach and gut upsets).
Various other drugs sometimes interact with antihistamines. For example, some antidepressants and some antifungal drugs. Therefore, if you are taking other medication, before taking an antihistamine check with your doctor or pharmacist to see if there is a risk of an interaction. If you are taking an antihistamine you should avoid alcohol, as this may make drowsiness worse.

¿Cómo funcionan?

Las células del sistema inmunitario vigilan de manera constante la sangre y las mucosas que recubren las cavidades del organismo, como la boca, la nariz o el tubo digestivo de cualquier cosa que no fabrique el cuerpo: por ejemplo, los microbios, como los virus y las bacterias. Si la piel se le ha dañado o el sistema inmunitario detecta una sustancia extraña, los mastocitos liberan la histamina que se une a los receptores específicos de otras células, denominados H1. Esto desata una reacción en cadena que provoca que los vasos sanguíneos en la zona se vuelvan ligeramente permeables. En la mayoría de la población, la respuesta inmunitaria es normal y adecuada, pero en aquellos que padecen alergia, es desmedida. Este exceso provoca síntomas, como prurito, hinchazón, ojos llorosos, etc.
Los antihistamínicos actúan físicamente bloqueando los receptores H1 e impiden que la histamina logre su objetivo. Esto disminuye la reacción del organismo a sustancias extrañas (alérgenos) y, como consecuencia, ayuda a reducir los síntomas molestos vinculados con la alergia.
También se utilizan para tratar las náuseas y los vómitos, aunque no se sabe con certeza la manera en que alivian estos síntomas. El cráneo tiene diversas áreas claves que controlan la acción de vomitar. 1. What Micardis is and what it is used for
Micardis belongs to a class of medicines known as angiotensin II
receptor antagonists. Angiotensin II is a substance produced in
your body which causes your blood vessels to narrow, thus increasing your blood pressure. Micardis blocks the effect of angiotensin II so that the blood vessels relax, and your blood pressure is lowered.
Micardis is used to treat essential hypertension (high blood pressure) in adults. ‘Essential’ means that the high blood pressure is not caused by any other condition.
High blood pressure, if not treated, can damage blood vessels in several organs, which could lead sometimes to heart attack, heart or kidney failure, stroke, or blindness. There are usually no symptoms of high blood pressure before damage occurs. Thus it is important to regularly measure blood pressure to verify if it is within the normal range.
Micardis is also used to reduce cardiovascular events (i.e. heart attack or stroke) in adults who are at risk because they have a reduced or blocked blood supply to the heart or legs, or have had a stroke or have high risk diabetes. Your doctor can tell you if you are at high risk for such events.
2. What you need to know before you take Micardis Do not take Micardis
• if you are allergic to telmisartan or any other ingredients of this medicine (listed in section 6).
• if you are more than 3 months pregnant. (It is also better to avoid Micardis in early pregnancy – see pregnancy section.)
• if you have severe liver problems such as cholestasis or biliary obstruction (problems with the drainage of the bile from the liver and gall bladder) or any other severe liver disease.
• if you have diabetes mellitus or impaired kidney function and you are treated with Rasilez.
If any of the above applies to you, tell your doctor or pharmacist before taking Micardis.
Warnings and precautions
Talk to your doctor if you are suffering or have ever suffered from any of the following conditions or illnesses:
• Kidney disease or kidney transplant.
• Renal artery stenosis (narrowing of the blood vessels to one or both kidneys).
• Liver disease.
• Heart trouble.
• Raised aldosterone levels (water and salt retention in the body along with imbalance of various blood minerals).
• Low blood pressure (hypotension), likely to occur if you are dehydrated (excessive loss of body water) or have salt deficiency due to diuretic therapy (‘water tablets’), low-salt diet, diarrhoea, or vomiting.
• Elevated potassium levels in your blood.
• Diabetes.
Talk to your doctor before taking Micardis:
• if you are taking Rasilez, a medicine used to treat high blood pressure.
• if you are taking digoxin.
You must tell your doctor if you think you are (or might become) pregnant. Micardis is not recommended in early pregnancy, and must not be taken if you are more than 3 months pregnant, as it may cause serious harm to your baby if used at that stage (see pregnancy section).
In case of surgery or anaesthesia, you should tell your doctor that you are taking Micardis.
Micardis may be less effective in lowering the blood pressure in black patients.
Children and adolescents
The use of Micardis in children and adolescents up to the age of 18 years is not recommended.
Other medicines and Micardis
Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines. Your doctor may need to change the dose of these other medicines or take other precautions. In some cases you may have to stop taking one of the medicines. This applies especially to the medicines listed below taken at the same time with Micardis:
• Lithium containing medicines to treat some types of depression.
• Medicines that may increase blood potassium levels such as salt substitutes containing potassium, potassium-sparing diuretics (certain ‘water tablets’), ACE inhibitors, angiotensin II receptor antagonists, NSAIDs (non steroidal anti-inflammatory medicines, e.g. aspirin or ibuprofen), heparin, immuno- suppressives (e.g. cyclosporin or tacrolimus), and the antibiotic trimethoprim.
• Diuretics (‘water tablets’), especially if taken in high doses together with Micardis, may lead to excessive loss of body water and low blood pressure (hypotension).
• Rasilez, a medicine used to treat high blood pressure.
• Digoxin.
The effect of Micardis may be reduced when you take NSAIDs (non steroidal anti-inflammatory medicines, e.g. aspirin or ibuprofen) or corticosteroids.
Micardis may increase the blood pressure lowering effect of other medicines used to treat high blood pressure or of medicines with blood pressure lowering potential (e.g. baclofen, amifostine). Furthermore, low blood pressure may be aggravated by alcohol, barbiturates, narcotics or antidepressants. You may notice this as dizziness when standing up. You should consult with your doctor if you need to adjust the dose of your other medicine while taking Micardis.
Pregnancy and breast-feeding
Pregnancy
You must tell your doctor if you think you are (or might become) pregnant. Your doctor will normally advise you to stop taking Micardis before you become pregnant or as soon as you know you are pregnant and will advise you to take another medicine instead of Micardis. Micardis is not recommended in early pregnancy, and must not be taken when more than 3 months pregnant, as it may cause serious harm to your baby if used after the third month of pregnancy.
Breast-feeding
Tell your doctor if you are breast-feeding or about to start breast-feeding. Micardis is not recommended for mothers who are breast-feeding, and your doctor may choose another treatment for you if you wish to breast-feed, especially if your baby is newborn, or was born prematurely.
Driving and using machines
Some people feel dizzy or tired when taking Micardis. If you feel dizzy or tired, do not drive or operate machinery.
Micardis contains sorbitol.
If you are intolerant to some sugars, consult your doctor before taking Micardis.
3. How to take Micardis
Always take Micardis exactly as your doctor has told you. Check with your doctor or pharmacist if you are not sure.
The recommended dose of Micardis is one tablet a day. Try to take the tablet at the same time each day.
You can take Micardis with or without food. The tablets should be swallowed with some water or other non-alcoholic drink. It is important that you take Micardis every day until your doctor tells you otherwise. If you have the impression that the effect of Micardis is too strong or too weak, talk to your doctor or pharmacist.
For treatment of high blood pressure, the usual dose of Micardis for most patients is one 40 mg tablet once a day to control blood pressure over the 24-hour period. Your doctor has recommended a lower dose of one 20 mg tablet daily. Micardis may also be used in combination with diuretics (‘water tablets’) such as hydrochlorothiazide which has been shown to have an additive blood pressure lowering effect with Micardis.
For reduction of cardiovascular events, the usual dose of Micardis is one 80 mg tablet once a day. At the beginning of the preventive therapy with Micardis 80 mg, blood pressure should be frequently monitored.
If your liver is not working properly, the usual dose should not exceed 40 mg once daily.

If you accidentally take too many tablets, contact your doctor, pharmacist, or your nearest hospital emergency department immediately.
If you forget to take Micardis
If you forget to take a dose, do not worry. Take it as soon as you remember then carry on as before. If you do not take your tablet on one day, take your normal dose on the next day. Do not take a double dose to make up for forgotten individual doses.

4. Possible side effects
Like all medicines, this medicine can cause side effects, although not everybody gets them.
Some side effects can be serious and need immediate medical attention:
You should see your doctor immediately if you experience any of the following symptoms:
Sepsis* (often called “blood poisoning”, is a severe infection with whole-body inflammatory response), rapid swelling of the skin and mucosa (angioedema); these side effects are rare (may affect up to 1 in 1,000 people) but are extremely serious and patients should stop taking the medicine and see their doctor immediately. If these effects are not treated they could be fatal.

What Micardis contains
The active substance is telmisartan. Each tablet contains 20 mg telmisartan.
The other ingredients are povidone, meglumine, sodium hydroxide, sorbitol (E420) and magnesium stearate.
What Micardis looks like and contents of the pack
Micardis 20 mg tablets are white, round and engraved with the code number ‘50H’ on one side and the company logo on the other side.
Micardis is available in blister packs containing 14, 28, 56, or 98 tablets.
Not all pack sizes may be marketed in your country.

Common side effects (may affect up to 1 in 10 people):
Low blood pressure (hypotension) in users treated for reduction of cardiovascular events.
Uncommon side effects (may affect up to 1 in 100 people): Urinary tract infections, upper respiratory tract infections (e.g. sore throat, inflamed sinuses, common cold), deficiency in red blood cells (anaemia), high potassium levels, difficulty falling asleep, feeling sad (depression), fainting (syncope), feeling of spinning (vertigo), slow heart rate (bradycardia), low blood pressure (hypotension) in users treated for high blood pressure, dizziness on standing up (orthostatic hypotension), shortness of breath, cough, abdominal pain, diarrhoea, discomfort in the abdomen, bloating, vomiting, itching, increased sweating, drug rash, back pain, muscle cramps, muscle pain (myalgia), kidney impairment including acute kidney failure, pain in the chest, feeling of weakness, and increased level of creatinine in the blood.
Rare side effects (may affect up to 1 in 1,000 people): Sepsis* (often called “blood poisoning”, is a severe infection with whole-body inflammatory response which can lead to
death), increase in certain white blood cells (eosinophilia), low platelet count (thrombocytopenia), severe allergic reaction (anaphylactic reaction), allergic reaction (e.g. rash, itching, difficulty breathing, wheezing, swelling of the face or low blood pressure), low blood sugar levels (in diabetic patients), feeling anxious, somnolence, impaired vision, fast heart beat (tachycardia), dry mouth, upset stomach, abnormal liver function (Japanese patients are more likely to experience these side effect), rapid swelling of the skin and mucosa which can also lead to death (angioedema also with fatal outcome), eczema (a skin disorder), redness of skin, hives (urticaria), severe drug rash, joint pain (arthralgia), pain in extremity, tendon pain, flu-like-illness, decreased haemoglobin (a blood protein), increased levels of uric acid, increased hepatic enzymes or creatine phosphokinase in the blood.
Very rare side effects (may affect up to 1 in 10,000 people): Progressive scarring of lung tissue (interstitial lung disease)**.
* The event may have happened by chance or could be related to a mechanism currently not known.
**Cases of progressive scarring of lung tissue have been reported during intake of telmisartan. However, it is not known whether telmisartan was the cause.
If you get any side effects, talk to your doctor or pharmacist. This includes any possible side effects not listed in this leaflet.
5. How to store Micardis
Keep this medicine out of the sight and reach of children.
Do not use this medicine after the expiry date which is stated on the carton after “EXP”. The expiry date refers to the last day of that month.
This medicine does not require any special temperature storage conditions. You should store your medicine in the original package in order to protect the tablets from moisture. Remove your Micardis tablet from the blister only directly prior to intake.
Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help protect the environment.

1. Qué es Micardis y para qué se utiliza
Pertenece a una clase de medicamentos que se denominan antagonistas de los receptores de la angiotensina II; una sustancia que se fabrica en el organismo y que provoca el estrechamiento de los vasos sanguíneos y, como consecuencia, aumenta la presión arterial. Este fármaco bloquea el efecto de esta sustancia de manera que los vasos se distienden y la presión desciende.
Micardis se utiliza para curar la hipertensión esencial (presión arterial elevada) en adultos; «esencial» significa que la causa no se debe a otra enfermedad.
Si esta alteración no se atiende, puede dañar los vasos sanguíneos en varios órganos y conducir a un infarto del miocardio, insuficiencia cardíaca o renal, accidente cerebrovascular o amaurosis (ceguera). Por lo general, la hipertensión no presenta síntomas antes de que se produzca la lesión; por lo tanto, es importante determinarla con regularidad para comprobar que se encuentre dentro del intervalo normal.
Micardis también se emplea para reducir la frecuencia de los episodios cardiovasculares (es decir, infarto del miocardio y accidente cerebrovascular) en la población adulta que se encuentra en peligro debido a una irrigación sanguínea deficiente en los miembros inferiores o en el corazón o que ha presentado un ictus o tiene una probabilidad elevada de padecer diabetes. El especialista le indicará si es vulnerable a cualquiera de estas circunstancias.
2. Qué necesita saber antes de empezar a tomar Micardis
No tome Micardis:
• si es alérgico al telmisartán o a cualquier otro ingrediente de este medicamento (se enumeran en el apartado 6).
• Si cursa más de 3 meses de embarazo (es mejor evitarlo durante el primer trimestre, consulte el apartado Embarazo).
• Si padece afecciones hepáticas graves, como colestasis u obstrucción biliar (dificultades con el drenaje de la bilis del hígado y de la vesícula biliar) o cualquier otra enfermedad grave de esos órganos.
• Si padece diabetes sacarina o deficiencia renal funcional o recibe aliskireno.
Si su caso es alguno de los mencionados, coménteselo a su médico o farmacéutico.
Advertencias y precauciones
Comuníquese con el especialista si padece o padeció algunas de las siguientes enfermedades o alteraciones:
• enfermedad o trasplante renal.
• Estenosis arterial renal (estrechamiento de los vasos sanguíneos hacia uno o ambos riñones).
• Cardiopatía.
• Hiperaldosteronismo (retención hidrosalina junto con un desequilibrio electrolítico).
• Hipotensión arterial; es factible que le suceda si está deshidratado o con deficiencia de sodio debido al tratamiento con diuréticos, dieta hiposódica, diarrea o vómitos.
• Aumento de las concentraciones de potasio en la sangre.
• Diabetes.
Hable con su médico antes de tomarlo
• Si se le administra aliskireno (fármaco que se utiliza para atender la hipertensión).
• Si toma digoxina.
También debe comentarle si cree que quedó o (pudiera quedar) embarazada. Se desaconseja tomar este medicamento al comienzo del embarazo y, no debe hacerlo, si ya pasó el primer trimestre, ya que puede causarle daños graves al feto (consulte el aparato Embarazo). De igual modo, en el caso de someterse a una intervención quirúrgica o anestesia. Es posible que este fármaco sea menos eficaz en lograr el descenso de la presión arterial en pacientes de raza negra.
Población pediátrica (niños y adolescentes)
Se desaconseja el uso de Micardis en pacientes pediátricos, es decir, menores de 18 años.
Interacción con otros medicamentos
Coméntele al especialista o farmacéutico si está tomando, tomó o podría tomar otros medicamentos. Puede que le tengan que cambiar la dosis o adoptar otras precauciones. En algunos casos hasta le pueden indicar dejar de tomar alguno de ellos. Esto concierne, en especial, a los fármacos enumerados a continuación que se ingieren al mismo tiempo que Micardis:
• Los medicamentos que contienen litio para curar ciertos tipos de trastornos depresivos.
• Los fármacos que pueden aumentar las concentraciones de potasio, como los sustitutos de la sal que lo contengan: los diuréticos ahorradores de potasio, los inhibidores IECA, los antagonistas de los receptores de la angiotensina II, los AINE (antinflamatorios no esteroideos, como la el ácido acetilsalicílico o el ibuprofeno), la heparina, los inmunosupresores (por ejemplo, la ciclosporina o el tracolimus) y el antinfeccioso trimetropina.
• Los diuréticos, en especial, si se toman en dosis elevadas junto con Micardis, pueden conducir a una deshidratación e hipotensión.
• El aliskireno, un fármaco que se utiliza para tratar la hipertensión arterial.
• La digoxina.
El efecto de Micardis puede disminuir cuando toma AINE (antinflamatorios no esteroideos, como ácido acetilsalicílico o el ibuprofeno) o corticoides.
Micardis puede aumentar el efecto hipotensor de otros medicamentos que se utilizan para tratar la hipertensión arterial o de fármacos con un posible efecto hipotensor (por ejemplo, el baclofeno y la amifostina). Además, la hipotensión arterial se puede agravar con el consumo de alcohol, barbitúricos, estupefacientes o antidepresivos. Lo puede notar cuando se quiere poner de pie y se marea. Debe consultar con su médico si necesita modificar la dosis de los otros medicamentos mientras reciba Micardis.
Embarazo y lactancia materna
Embarazo
Debe comentarle al médico si cree que quedó o (pudiera quedar) embarazada. Por lo general, le aconsejará que deje de tomar Micardis antes o apenas lo sepa y le recetará otro medicamento. Se desaconseja tomarlo al comienzo del embarazo y no debe hacerlo si ya pasó el primer trimestre, ya que puede causarle daños graves al feto.
Lactancia materna
También es importante avisarle si está amamantando o a punto de empezar. En ese caso, se desaconseja el uso del medicamento. Puede elegir otro tratamiento si desea dar el pecho, en especial, a recién nacidos o prematuros.
Conducción y uso de máquinas
Algunas pacientes suelen marearse o sentirse cansados cuando toman Micardis. Si es el caso, no conduzca ni maneje máquinas.
Este medicamento contiene sorbitol
Si es intolerante a algunos azúcares, consulte con su médico.
3. Cómo tomar Micardis
Hágalo siempre exactamente como le dijo el especialista. Consúltele, si no está seguro.
La dosis recomendada es un comprimido diario, en lo posible, a la misma hora.
Puede tomarlo con o sin comida y tragarlo con un vaso de agua u otra bebida sin alcohol. Es importante que lo haga todos los días hasta que su médico le indique lo contrario. Si considera que el efecto es muy potente o inconsistente, coménteselo.
Para el tratamiento de la hipertensión arterial, la dosis habitual para la mayoría de los pacientes es un comprimido diario de 40 mg para controlar la presión arterial durante un período de 24 horas. Se le ha recomendado una dosis menor de un comprimido diario de 20 mg. Micardis también se puede administrar en forma concomitante con diuréticos, como la hidroclorotiacida que ha demostrado tener un efecto hipotensor.
Para disminuir la aparición de los accidentes cerebrovasculares, la dosis normal es un comprimido diario de 80 mg. Al comienzo del tratamiento preventivo y, con esa dosificación, se debe vigilar la presión arterial con frecuencia.
Si tiene problemas hepáticos, la toma diaria habitual no debe superar los 40 mg.
Si toma más Micardis del que debe
Sí, de manera accidental, toma demasiados comprimidos, póngase en contacto con su médico o farmacéutico o diríjase de inmediato a Urgencias del hospital más cercano.
Si se olvidó de tomar Micardis
Sí este fuera el caso, no se preocupe. Tómelo en cuanto se acuerde y luego continué como de costumbre. No ingiera una dosis doble para compensar, sino la que le corresponde al día siguiente.

4. Posibles efectos adversos
Como sucede con todos los medicamentos, este también puede provocar efectos adversos, aunque no todos los pacientes los padezcan.
Algunos son graves y necesitan atención médica inmediata:
Si nota algunos de los siguientes síntomas, debe acudir a su médico con urgencia:
Síndrome séptico (por lo general, se le llama «septicemia», es una infección grave que implica una respuesta inflamatoria de todo el organismo), inflamación rápida de la piel y la mucosa (angioedema); estos efectos son poco frecuentes (puede afectar a 1 de cada 1000 personas), pero extremadamente graves. Los pacientes deben suspender el medicamento y acudir al médico de inmediato. Si no se tratan, pueden ser mortales.
Efectos adversos muy frecuentes (pueden afectar a 1 de cada 10 pacientes):
Hipotensión arterial (presión arterial baja) en pacientes que se atienden para disminuir la frecuencia de los episodios cardiovasculares.
Efectos adversos poco frecuentes (pueden afectar a 1 de cada 100 pacientes):
Infección urinaria, infección de las vías respiratorias superiores (por ejemplo: faringitis, sinusitis, resfriado), anemia (falta de glóbulos rojos), concentración elevada de potasio, dificultad para conciliar el sueño, depresión, desmayo, vértigo, bradicardia (frecuencia cardíaca lenta), hipotensión arterial en pacientes con hipertensión, mareos al ponerse de pie (hipotensión ortostática), disnea, tos, dolor abdominal, diarrea, incomodidad abdominal, flatulencia, vómitos, prurito, aumento de la transpiración, sarpullido, dorsalgia (dolor de espalda), calambres musculares, mialgia (dolor localizado en uno o varios músculos), insuficiencia renal, dolor torácico, debilidad, aumento de la concentración de la creatinina.
Efectos adversos raros (pueden afectar a 1 de cada 1 000 pacientes):
Síndrome séptico (por lo general, se le llama septicemia, es una infección grave que implica una respuesta inflamatoria de todo el organismo y puede ser mortal), aumento de ciertos glóbulos blancos (eosinofilia), disminución de plaquetas (trombocitopenia), reacción alérgica grave (reacción anafiláctica), reacción alérgica (sarpullido, prurito, dificultad para respirar, sibilancias, hinchazón en el rostro e hipotensión arterial), hipoglucemia (en pacientes diabéticos), ansiedad, somnolencia, deficiencia visual, taquicardia, sequedad de boca, malestar epigástrico, anomalías en la actividad renal (es más probable que los japoneses suelan padecer este tipo de efectos), inflamación rápida de la piel y la mucosa que también puede conducir a la muerte (angioedema con un desenlace mortal), eccema, alteración dérmica, eritema, urticaria, sarpullido grave, dolor en las articulaciones (artralgias), dolor en las extremidades inferiores y en el tendón, síndrome seudogripal, descenso de la hemoglobina (una proteína de la sangre), aumento de los valores de ácido úrico, incremento de las enzimas hepáticas (creatina-cinasa).
Efectos adversos muy raros: (pueden afectar a 1 de cada 10 000 pacientes):
Deformidad cicatricial progresiva del tejido pulmonar (fibrosis pulmonar).
El episodio pudo haber sucedido por casualidad o puede estar asociado con un mecanismo patogénico desconocido hasta el momento.
Se informaron casos de fibrosis pulmonar durante el consumo del telmisartán, aunque no se sabe si fueron provocados por el medicamento.
Si padece algún efecto adverso, consulte con su médico o farmacéutico, incluso si no está detallado en el prospecto.
5. Conservación de Micardis
Mantenga este fármaco fuera del alcance los niños.
No utilice este medicamento después de la fecha de vencimiento que aparece en el envase luego de la leyenda «CAD», el plazo se refiere al último día de ese mes.
Micardis no necesita condiciones especiales de conservación de temperatura. Debe almacenar el medicamento en el envase original para proteger los comprimidos de la humedad. Sáquelo del emblistado directamente antes de tomarlo.
No tire el medicamento en el desagüe o a la basura. Pregunte al farmacéutico como desechar fármacos que ya no utiliza. Estas medidas ayudarán a proteger el medio ambiente
6. Contenido del envase e información adicional
Composición de Micardis
El principio activo es el telmisartán y cada comprimido contiene 20 mg. Los demás componentes son la povidona, la meglumina, el hidróxido de sodio, el sorbitol (E-420) y el estereato de magnesio.
Aspecto del producto y contenido del envase
Los comprimidos de 20 mg son blancos, redondos y llevan grabado el código «50H» en un lado y el logotipo de la empresa en el otro. Micardis se fabrica en envases emblistados que contienen 14, 28, 56 o 98 comprimidos.
Es posible que en su país no se dispongan de todos los tamaños del envase.
