Der Einsatz von Rate-Smoothing auf Einkammerebene soll am Fall eines Patienten mit belastungsabhängigem SA-Block und paroxysmalem atrialen Flattern/Flimmern sowie dadarch bedingten synkopischen Episoden demonstriert werden.
Der begeisterte Tennisspieler und Jogger klagte 1984 erstmalig über Dyspnoe und kurze Schwindelattacken während einer Belastung. Im Langzeit-EKG fand man untcr Belastung Episoden von paroxysmalem atrialen Flattern mit einem 2:1-Block, welches sofort nach Beenden der Aktivität in einen Sinus-Rhythmus konvertierte. 1n der Ergometric wurden normale Blutdruckwerte und initial eine chronotrope Suffizienz beobachtet. Allerdings trat bei hohen Belastungsstulen intermittierendes atriales Flattern auf.
Als die Inzidenz der Arrhythmie zunahm, wurde ein stabilcr Sinus-Rhythmus durch die Gabe von Digitalis und Verapamil erreicht. In den nächsten zwei Jahren fühlte sich der Patient wohl, und erkonnte seinen Sporttuten sowie seinen täglichen Aktivitäten ohne Einschränkung nachgehen.
Seit 1988 klagte der Palient erneut über Dyspnoe, Schwindel und zusätzlich über eine Reduktion seiner physischen Leistungskapazität.
In wiederholten Ergometrien fand man Sinus-Rhythinns und Normolonie bei maximaler Belastung. Im Langzeit-EKG jedoch konnten die beobachteten Vorhof-Arrhythmien wieder festgestellt werden. Die Durchuntersuchung ergab keinerlei Hinweis auf KHK, Vitium oder Kardiomyopathie. Da der Patient seine Medikation selbst abgesetzt hatte, wurde eine erneute Therapie mit sich einstellendem scheinbaren Erfolg anegsetzt.
Im Oktober 1989 erlitt dcr Patient zwei kurze Synkopen jeweils am Ende eines Tennisspicls. Im Langzeit-EKG wurden bradykarde Episoden (32/min) sowie intermittierendes atriales Flimmern mit Pausen bis zu 3 s beobachter. Die maximale Herzfrequenz stieg kurzfristig bis auf 170/min an. Während der Ergometrie waren Herzfrequenz und Blutdruck normal, aber in der Erholungsphase kam es plötzlich zum Abfall der Sinus-Frequenz von 140/min auf 40/min. Gleichzeitig wurde der Patient bleich und schwindlig, hatte jedoch keine Zeichen von Angina pectoris. Eine Digitalis-Intoxikation konnte ausgeschlosscn werden (der Patient hatte erneut seine Medikation abgesetzt).

Previous history
Using rate-smoothing at the single-chamber level should be demonstrated in the case of a patient with the exertion-related SA block and paroxysmal atrial flutter/fibrillation as well as thus caused syncopic episodes. An enthusiastic tennis player and jogger, the patient, for the first time, complained about dyspnea and brief vertiginous attacks during exertion. In the prolonged ECG, under exertion, we found episodes of paroxysmal atrial flutter with a 2:1 block, which converted to a sinus-rhythm immediately after the activity. Normal blood pressure values and initially chronotropic sufficiency were observed in the ergometry. However, intermittent atrial flutters occurred during high degrees of exertion. As the arrhythmia incidence increased, a stable sinus-rhythm was achieved by giving digitalis and verapamil. In the next two years, the patient remained without problems and he could follow his sport activities along with his daily activities without any limitations. Since 1988 the patient has been complaining about dyspnea, vertigo and, additionally, about a decrease in his physical performance.
In renewed ergometries we found sinus-rhythm and normolony at maximum exertion. However, the earlier observed atrial arrhythmias could again be determined in the prolonged ECG. The continuous examination did not show any sign of coronary heart disease, defect or cardiomyopathy. As the patient had discontinued the medicines himself, a renewed therapy was started with apparent possible success. In October 1989 the patient suffered two brief syncopes at the end of a tennis play. Bradycardiac episodes (32/minute) and an intermittent atrial fibrillation with pauses of up to 3 s were observed in the prolonged ECG. Maximum cardiac frequency and blood pressure were normal but in the resting phase the patient suddenly experienced the descent in sinus frequency from 140/minute. At the same time the patient became pale and vertiginous but did not have any signs of angina pectoris. A digitalis intoxication could be ruled out (the patient had again stopped medicines).
