In der Abwägung ist aus medizinischer und versorgerischer Sicht die frühzeitige Erkennung und Therapie aktinischer Keratosen und damit die Abwendung zunehmender Krankheitslast (u.a. Lebensqualität, Kosten) notwendig. Wegen ihrer geringen Invasivität und hohen Akzeptanz bei den Patienten kommt den topischen Arzneimittelanwendungen eine besondere Bedeutung zu, insbesondere bei der Feldkanzerisierung und der damit verbundenen Flächentherapie. Voraussetzung für deren Anwendung ist die sachgerechte Diagnostik der Läsionen, welche in erster Linie durch den Dermatologen erfolgen sollte. In Ergänzung zur klinischen Diagnostik können hier dermatohistopathologische Untersuchungen notwendig sein.

An actinic keratosis (AK) is a pre-malignant cutaneous lesion that frequently manifests in sun-exposed areas of the skin as a small, rough, scaly erythematous papule. They are one of the most common presenting complaints for dermatologists. AKs should be treated due to their potential to progress into a squamous cell carcinoma (SCC). There are numerous treatments available for managing AKs including those broadly categorized as destructive, topical field, and procedural field therapies. The topical field therapies include 5-fluorouracil, imiquimod, and diclofenac gel. Recently, imiquimod 3.75% (Zyclara™) has been approved for the treatment of AKs on the face and scalp. It is a reasonable alternative to imiquimod 5%, as the approved indication includes a larger surface area for treatment, shorter duration course, and the potential for less severe local skin reactions. There is no widely accepted algorithm for the treatment of AKs, as comparative data is unavailable between all approaches. Therapy choices are guided by efficacy, adverse effects, cosmetic results, and patient compliance.

PALM BEACH, FLA. -- Field therapy for actinic keratosis with imiquimod is noninvasive, improves cosmetic outcome, and can be adapted as short-course cycle therapy rather than being given for the full 16-week recommended regimen.

Field therapy involves the application of a topical agent over large areas of skin, rather than just to the area of the lesion. It is based on the concept of "field cancerization," which initially referred to the observation that subclinical disease was often found around areas of tumors in the gastrointestinal mucosa.

The definition of field cancerization has evolved to include clinically occult but multifocal preneoplastic changes associated with genetic mutations that precede the development of second primary tumors or recurrences, explained Dr. Neal A. Fenske, who is chairman and professor of dermatology and cutaneous surgery, University of South Florida, Tampa.

With actinic keratosis (AK), the goal of field therapy is to eliminate mutated keratinocytes that have the potential to become squamous cell carcinomas.

Besides imiquimod (Aldara), other agents currently used as field therapy are 5-fluorouracil (5-FU) and diclofenac, Dr. Fenske said at the annual meeting of the Florida Society of Dermatologic Surgeons.

As experience with imiquimod for AKs has increased, it has become clear that the treatment regimen can be shortened and modified to maintain efficacy and minimize side effects.

"Cycle therapy is a new and novel way to use imiquimod, with treatment three times per week for 4 weeks followed by a 4-week rest period," Dr. Fenske said.

The patient is then assessed and, if visible AKs are still present, a second cycle of treatment can be given, he said.
