The patient in this series of reports iis described as having an "infiziertes Hämatom Oberschenkel Innenseite links", and the treatment summary reads "Indikation zur notfallmäßigen OP, *Hämatomspaltung*"
There are references elsewhere in the report to Hämatomausräumung, but this is the only time "Spaltung" is mentioned, and I'd like to be clear about what is actually being done here - is it literally being split, or simply "separated/removed" in this case?

What is being done in Hämatomspaltung is an incision of the overlying tissues (not really "incision of hematoma", as the word might suggest).
This (common) German concept spells out the incision and implies the drainage, whereas English reports would commonly spell out "drainage" and often omit/imply the "incision" part.

...is the wording I would suggest you use in your context, where "the treatment summary reads Indikation zur notfallmäßigen OP, Hämatomspaltung".
Removal of hematoma would be Hämatomausräumung, as already mentioned elsewhere in your source text. Das Hämatom spalten means to cut open the hematoma (surgically). So, in your context, Hämatomspaltung means that the hematoma was cut open (surgically). This idea is better expressed in medical (or, should I say, surgical?) parlance as surgical evacuation of the hematoma.
Here is an example of the usage of the term in a context perhaps similar to yours:
Diagnosis: Often evident from an athlete reporting a direct blow to the thigh followed ... aid in determining when to consider surgical evacuation of the hematoma.
