Whenever I've had a misbehaving TM or TB, I would repair it. Some with a project. Unlike other CAT tools, DVX3 lets you repair projects, translation memories and termbases. I would suggest your colleague to get in that habit.

It could be mismatched language variants

Silly me encountered a similar situation in the past whenever I used Spain-Spanish in one TM and US Spanish in a similar TM. What languages are your team using then?
I know most project managers show an appalling incompetence when talking about languages for a project: they say they need a French translator but you have to poke them in the sides to reveal which French variety: Canadian, Parisian, etc.
