No urgency in this question. How would you translate "Nervenarzt"? I used "neurologist" and below is the comment from the client:

"Noch eine Bemerkung zu Ihrer Übersetzung des Arbeitszeugnises. Sie hatten dort "Nervenarzt" mit "neurologist" übersetzt.
Dies ist nicht ganz korrekt, ist aber für meinen Zweck ohne Belang. Falls sie nochmal mit dem Begriff konfrontiert werden:
Der Neurologe (neurologist) kümmert sich um organische Hirnerkrankungen. zB Schlaganfall, Parkinson, MS, Epilepsie.
Der Psychiater (psychiatrist) ist zuständig für die Krankheiten, die organisch nicht so eindeutig zu erklären sind, wie Schizophrenie, Depression, Angststörungen.
Der Nervenarzt ist ein eigenständiger Facharzt, der länger dauert als die beiden anderen und beide Fachgebiete umfasst.
Ein Nervenarzt behandelt somit psychiatrische und neurologische Erkrankungen. Diesen Facharzt gibt es allerdings, soweit ich weiß, nur noch in Deutschland. Alle anderen Länder haben ihn im Zuge der Spezialisierung abgeschafft.
Wie man ihn am Besten übersetzt weiß ich daher auch nicht. Vielleicht gibt es ja einen eher älteren Begriff im Englischen für den Neurologen, der wäre eventuell passender."

I would put both there. Die Nervenärzte in Deutschland haben meist auch auf ihren Schildern Neurologe/Psychiater stehen, denn ein Nervenarzt ist ein Facharzt für Neurologie und Psychiatrie.

I think this is possibly the best that can be done. I certainly wouldn't use any old-fashioned English words (should they exists) unless the doctor wants to sound old-fashioned to create a special effect.
