Does that simply mean "nach Vereinbarung" or something completely different? I went into google.ch and found it there several times without any pointers on what it means.

Any suggestions?

One of three options in Beckers Abkuerzungslexikon medizinischer Begriffe. The others are Nach Verabreichung and neue Variante. Given the fact the coumadin levels charge and are frequently adjusted over the phone after the labs are drawn, I think that this signifies that the patient should follow the doctor's verbal orders on a long-term basis rather than slavishly adhere to what the bottle says. This also alerts emergency personal that the dose may not be exactly what is on the bottle.

Da es sich offenbar um die "letzte Medikation" bzw. die Medikationsempfehlung für den Zeitraum nach Entlassung handelt, könnte ".n.V." hier auch "nach Verordnung" - 'by/dependent on/in accordance to (individual) prescription' - heißen...
